Your form lives inside Shopify metaobjects until you place the block on a theme page. This is the step that makes the form visible to customers.
How to add the block
In your Shopify admin, go to Online Store → Themes → Customize on your live theme.
Navigate to the page where you want the form (home page, contact page, product page — any page works).
Click Add section or Add block in the area you want the form.
Pick the Apps tab and select StoreForms form.
In the block settings on the right, pick which form to render from the dropdown.
Click Save in the top right of the theme editor.
Tips
The same form can be embedded on multiple pages — useful for a sitewide contact form.
You can adjust width, alignment, and padding in the block settings without changing the form itself.
